Three-Day Nursing Documentation Training for John F. Kennedy Medical Center: (JFKMC)A three-day training on Nursing Documentation was held for nurses of the John F. Kennedy Medical Center (JFKMC) and the E.S. Grant hospital.
Providing an overview of the training, Dr. John Emmanuel Tamba, Chief Medical Officer of JFKMC, emphasized that documentation is essential for delivering detailed, accurate, and objective patient care. He encouraged the nurses to consistently record assessments, vital signs, medications, incident reports, and use flowsheets for specific interventions to ensure continuity and quality of care. Dr. Tamba reminded the nurses that, as caregivers, they are responsible for closely observing patients’ conditions and documenting their findings.
During the morning session, Mrs. Joana D. Joekai, Director of Nursing, highlighted the significance of proper documentation. She stated that documentation provides a comprehensive, accurate, and timely record of patient care that supports communication, legal requirements, continuity, and quality assurance within the healthcare setting.
Mrs. Joekai urged nurses to document all patient information in detail. She referenced documentation standards from the Liberia Medical and Dental Council (LMDC), African nursing documentation practices, and current JFKMC protocols.
Speaking during the training, Dr. Linda A. Birch, Chief Executive Officer of JFKMC, acknowledged the training team and stressed the importance of documentation. She emphasized that writing detailed of patient notes is not just a task but a professional responsibility. You may not be earning much, but people are watching. Do your work with diligence and treat every patient as if they were your own child or relative,” she told the nurses that she expects to see measurable improvements in nursing documentation during the training, the way you treat patients and document record will open doors for future opportunities for you as a nurse.
One of the facilitators, Mrs. Ama A.A. Tarplah, a Nurse Anesthetist, reminded participants that documentation must be accurate, clear, complete, and timely.
Day Two – July 23, 2025
On the second day, Mrs. Ama A.A. Tarplah continued the training by introducing the FACTS model of documentation—Factual, Accurate, Complete, and Timely.
She also trained the nurses on documentation formats such as SOAP and PIE notes. According to her, SOAP stands for Subjective, Objective, Assessment, and Plan, while PIE stands for Problem, Intervention, and Evaluation. She explained how these methods can help structure patient records and improve communication among healthcare teams.
Day Three – July 24, 2025
Today, July 24, 2025, marks the climax of the special three-day training for nurses from the three hospitals under the John F. Kennedy Medical Center: (JFKMC). JFK Memorial Hospital, E.S. Grant Mental Health Hospital, and the Maternity Hospital.
The training took place at the JFK Memorial Hospital lobby and aimed to strengthen documentation practices among nurses by introducing tools like the SOAPIE method. It also sought to establish effective communication practices among nurses and other healthcare professionals at JFKMC.
The purpose of the training is to help nurses use the SOAPIE method and to establish effective communication tools among JFKMC nurses and healthcare professionals.
At the end of the program, the Director of Nursing Mrs. Joana D. Joekai encouraged nurses to be accurate, write complete notes, and ensure timely documentation.
